About

We have sat on both sides of this table.

Provenance Evaluations is an independent practice producing written medical evidence for disability claims. The name refers to the chain from source document to opinion: where a finding comes from, what supports it, and how a reviewer can retrace it.

Provenance Evaluations seal

Record strata threaded by one line of evidence, terminating in a documented finding.

Founder

Dr. Taylor English, Ph.D.

Dr. English is a licensed clinical psychologist. He earned his Ph.D. in clinical psychology from the University of Nebraska in 2021, after an M.A. in clinical psychology from Mississippi State University and a B.A. in psychology from the University of Arkansas.

He completed two years of doctoral and postdoctoral training at Department of Veterans Affairs medical centers, at the James A. Haley Veterans' Hospital in Tampa, Florida and at the Oklahoma City VA Health Care System. During those years he worked with veterans experiencing trauma and substance use disorders, and [conducted [N] compensation and pension examinations under the supervision of VA staff psychologists]. He watched well-founded claims fail for one reason: the medical evidence in the file did not say what the regulations required it to say. Provenance Evaluations exists to close that gap, one properly documented evaluation at a time.

Since 2022 Dr. English has practiced at Norman Behavioral Health Group in Norman, Oklahoma, where he serves as Training Director. His clinical work covers depression, anxiety, trauma, and substance use across adolescents and adults, using cognitive behavioral therapy, acceptance and commitment therapy, dialectical behavior therapy, relapse prevention, and cognitive processing therapy for post-traumatic stress disorder. He also conducts psychological assessments addressing attention, learning, intellectual, behavioral, and emotional questions.

License: Oklahoma, [license number, expiration]. Additional state licensure and PSYPACT status: [to confirm]. Malpractice and E&O coverage in force for forensic and disability evaluation work: [carrier, policy number].

Practice model

A small practice by design.

Every evaluation is conducted and signed by Dr. English. Reports are not delegated to unnamed associates or template-driven contractors. Volume is capped so that each file receives the records review, interview, and rationale it needs.

[If additional clinicians are added, list them here and update signing evaluator language throughout the site.]

Our standards
  • Every report signed by a licensed doctoral psychologist.
  • Fees disclosed before commitment; no fee tied to any outcome.
  • Opinions written only where the clinical evidence supports them.
  • Every record handled through a HIPAA-compliant portal under a BAA.
Licensure and coverage

Where we can evaluate.

Dr. English is licensed to practice psychology in Oklahoma. Telehealth evaluations are available to residents of Oklahoma and to residents of any state where he holds an active license or a compact privilege. [Update this list as additional state licensure and PSYPACT participation are added.]

Scope

The practice conducts psychological evaluations. It does not sign opinions on physical or orthopedic conditions, which fall outside a psychologist's scope of practice. For those, the referring representative should consult a physician evaluator.
